Individual Medical for Kids (up to 18 years of age)- MS Residency Required |
Individual Medical for Kids is an individual policy which covers office visits, in patient services, out patient services, emergency room services, hospital services and prescription. This policy is guaranteed issued, but only offered during special enrollment times during the year set by the carrier. For more information regarding Individual Medical for Kids, please contact us. |

Individual Medical for Adults (19+ of age)- MS Residency Required |
Individual Medical for Adults is an individual medical policy which covers office visits, in patient services, out patient services, emergency room services, hospital services and prescription. These policies are subject to underwriting. Approval or decline of applications is controlled by the carrier underwriter and based on medical records submitted. There are two plan designs to select from: Copay Provides an office visit copay, which does not count towards the annual deductible. Coinsurance No copay, which lowers your monthly premium. You pay 100% until you have reached your deductible.
Once your deductible has been met under either plan design, the carrier will pick up 80% of all costs for covered services at in network providers. |

Medicare Supplement Policy(65+ of age, unless declared disabled by SSA)- MS Residency Required |
Medicare Supplement plans allow you to have control of your out-of-pocket expenses. There are six supplement plans to choose from.
Before selecting a Medicare Supplement plan, you should read the Medicare publications "Medicare & You" and "Choosing a Medigap Policy" for explanations of Medicare benefits and Medicare Supplement insurance. The Medicare Supplement policies offered by Lockard & Williams do not include Drug Supplement plans. Please see your local pharmacist for assistance with Drug Supplement plans. |
