Major medical covers office visits, in patient services, out patient services, emergency room services, hospital services and prescription. There are countless plan designs to select from to suite the needs of each employer. Allow Lockard & Williams to assist you in selecting the best plan for you! Fully insured medical is generally used by small groups. Preliminary rates are provided from the carrier after submitting a census which includes age, sex and coverage. Once a carrier has been selected based on plan design and rates, employee applications (or employer addendum if applicable) with health questions are submitted to obtain final rates from the underwriter. Employers must contribute a minimum of 50-65% of the employee only cost, depending on the carrier regulations. Employees are responsible for up to 100% of dependent costs. Dental |
Many people do not realize their dental care is just as important as their medical care. Many health issues are related to dental, and can generally be prevented by maintaining dental screenings every six months. Our dental carriers provide routine check ups, basic services, major services as well as child and adult orthodontia. Let our office assist you in providing the dental care your employees deserve! Disability |
Short Term Disability and Long Term Disability are used by both small and large groups. This benefit allows an employee to feel at ease knowing their income in insured in the event they are placed on disability with no income accrual by the employer. Short Term Disability is typically extended to a maximum of 26 weeks after a 2 week elimination period. Long Term Disability is typically extended until the participant becomes able to work or reaches retirement age after a 26 week elimination period. Basic Life / AD&D |
Basic Life with Accidental Death & Dismemberment (AD&D) is used by both small and large groups. Amounts for the employee can range from $10,000 up to 3 times the annual salary of the employee, depending on the size of the group and carrier regulations. This benefit is typically paid 100% by the employer for all eligible employees. Preliminary rates are provided from the carrier after submitting a census which includes age, sex and coverage. Voluntary Term Life |
Voluntary Term Life is used by both small and large groups. Amounts for the employee can range from $10,000 up to 5 x annual salary of the employee. Dependent life is also available – Spouse is typically eligible for up to 50% of the employee election – Children are typically eligible for up to $20,000, depending on the size of the group and carrier regulations.
